Empyema (Infected Fluid Around the Lung)
Empyema means infected fluid or frank pus has collected around the lung. If the infection is not fully cleared, it turns into thick scar tissue that locks the lung shut.
What is empyema and why it forms
Empyema is a complication of pneumonia, chest trauma, surgery or tuberculosis where the pleural space becomes infected. The body tries to wall it off, but the pus thickens and tissues peel off the lung like a rind.
In the early phase the pus is thin and a simple drain may clear it. When it thickens into a peel (organised empyema), no amount of antibiotics drains it - the peel must be surgically removed in a procedure called decortication so the lung can expand again.
An empyema that is ignored can cause chronic illness, lung failure on that side, and even spread of infection. For that reason smart and timely surgery is the difference between a quick recovery and months of weakness.
The different forms
After pneumonia
Persistent infection or a lung abscess drains into the chest and turns into empyema.
After chest surgery
Sometimes a leak or contamination after surgery allows the chest to get infected.
After trauma
Blood left behind after an injury becomes a perfect breeding ground for infection.
Tuberculosis
Chronic TB of the pleura can produce a long-standing infected collection.

Dr Ahmad Ali explains the condition and surgery in simple words.
How you know you have it
Typical symptoms
- Fever that will not settle despite antibiotics
- Chest pain and heaviness on the affected side
- Shortness of breath and easy fatigue
- Sweats, chills and poor appetite
When to seek help urgently
- Fever with confusion or drowsiness
- Severe breathing difficulty
- Coughing up pus-coloured phlegm
- Rapid weight loss over weeks
How it is confirmed
Chest X-Ray
Shows fluid with a thickened lining that does not look like a simple effusion.
Ultrasound
Reveals pus, thick membranes or separated pockets (loculations) inside the chest.
CT Scan
Maps the exact stage of empyema and plans the safest surgical approach.
Pus Culture
Identifies the bacteria so the correct antibiotics run alongside surgery.
The procedures Dr Ahmad Ali performs
The stage of your empyema decides the fix - drain, keyhole clean, or open decortication.
Chest Drain + Antibiotics
- What it is — If the pus is still thin, a tube drain with strong targeted antibiotics often clears the infection.
- Eligibility — Fresh empyema with liquefied pus, before it turns thick.
- Stay — A few days in hospital on the drip.
- Recovery — Fever settles, chest stays drained, most patients regain appetite fast.
VATS Decortication
- What it is — Through keyhole cuts, infected material and membranes are peeled off and the lung is released.
- Eligibility — Organised empyema that is not too chronic; this is often the smartest first choice.
- Stay — Usually 3-5 days.
- Recovery — Breathing improves within days; drain comes out before discharge.
Thoracotomy & Decortication
- What it is — For long-standing, thick rinds - the peel is cleaned off through a carefully placed incision and the lung is fully expanded.
- Eligibility — Chronic empyema where keyhole cleaning is incomplete or the rind is very thick.
- Stay — 5-7 days in hospital.
- Recovery — Full lung re-expansion restores energy and breathing within a few weeks.
